The
purpose
of this
study
was to
evaluate
the
outcome
of
Intracytoplasmic
Sperm
Injection
(ICSI)
in
patients
with
previous
unexplained
fertilization
failure
(≤20%)
after
conventional
in
vitro
fertilization
(IVF)
in
our
program
and to
compare
them with those of
patients
undergoing
ICSI
for
male
factor
indications
. The
setting
was an
IVF
program
in a
University
center
with
retrospective
analysis
of
all
patients
treated
with
ICSI
for
previous
unexplained
fertilization
failure
.
Twenty-five
patients
who
underwent
thirty
eight
ICSI
cycles
after
experiencing
unexplained
fertilization
failure
with
conventional
IVF
comprised
group
A and
eighty
seven
patients
who
underwent
one
hundred
and
eighteen
ICSI
cycles
for
male
factor
indications
during
the
same
time
period
comprised
group
B
.
Fertilization
was
achieved
with
ICSI
in
all
patients
with
previous
fertilization
failure
.
Fertilization
rate
(68
±
21%
versus
64
±
22%)
,
implantation
rate/embryo
(22.6%
versus
20.0
%)
and
delivery
rate/cycle
(47.3%
versus
49.1
%)
did
not
differ
significantly
between
groups
A and
B
,
respectively
.
Overall
,
72%
of
patients
with
previous
unexplained
fertilization
failure
achieved
successful
pregnancy
after
ICSI
compared
to
67%
of
patients
with
male
factor
infertility
.
ICSI
can
overcome
unexplained
fertilization
failure
due
to a
potentially
occult
gamete
abnormality
,
achieving
the
same
fertilization
,
implantation
and
pregnancy
rates
as
found
in
patients
with
abnormal
semen
parameters
.
